The Department of Education requires our office to verify some financial aid applicants' citizenship/residency status. Those selected for verification must submit appropriate documentation, such as a birth certificate, certificate of naturalization, a valid U.S. passport, or United States Permanent Resident Card. Complete this form and submit it along with the documentation to the Office of Financial Aid via the My Financial Aid portal if requested.

Statement of Educational Purpose
The Department of Education randomly selects a portion of FAFSA applicants for verification. If you are notified by our office to submit this form, your record has been selected and you must submit the Statement of Educational Purpose and Proof of Identity form. Complete this form and submit it to the Office of Financial Aid via the My Financial Aid portal if requested.

Parent Tax Waiver Form
Complete this form and submit it to the Office of Financial Aid via the My Financial Aid portal if your parent(s) did not file taxes for the requested tax year. You should upload this form under the ‘Parent Tax Return’ link.
This form should not be submitted for a parent with whom you have no contact and cannot retrieve parent data. Please see below.
Parent Information Waiver Request
Complete this form and submit it to the Office of Financial Aid via the My Financial Aid portal if you do not have contact with your parent(s).
Heartland ECSI Deferment Form
Complete the Deferment Due to Continuing Education Studies Form and submit it to Heartland ECSI to defer University Loan payments during residency. Do not submit this form to the Office of Financial Aid. Our school code is 3D.
