Completing Your Financial Aid Application

Below you'll find the most common forms in use by the Office of University Financial Aid. Some of these are required by the office as part of a student's initial application for financial aid. Others are requested by the office on a case-by-case basis as documentation of special circumstances.

All information provided to the Office of University Financial Aid by the student, or by the student's parents or spouse, is completely confidential.

Although there are a large number of forms listed below, only a small number will apply to a given student. Please do not submit all these forms. As a general rule, if you don't know what the form is, or you're not certain it applies to you, please do not submit it.

These forms pertain to the 2024-25 academic year only. If you have any questions regarding the type of forms you should fill out, please do not hesitate to contact us.


ÐÇ¿Õ´«Ã½ Financial Aid Form

The ÐÇ¿Õ´«Ã½ Financial Aid form is required for all enrolling students. The form is available online through the My Financial Aid portal. In order to complete the form, log in using your ÐÇ¿Õ´«Ã½ Network ID.

Dislocated Worker Verification

Please only submit this form if it is listed as a required document on your My Financial Aid portal. This form verifies a parent’s status as a dislocated worker according to federal guidelines.

Clarification Form (Parents)

Please only submit this form if you have been contacted by financial aid staff that advised you of which portion of the form to complete. Only those items requested need to be completed.

Memorandum of Assistance (MOA)

The Memo of Assistance form is required of all graduate and professional students applying for financial aid. Unlike most other forms, it is not completed by the student. Instead, it is completed by the graduate department for the program in which the student is enrolled. A graduate department representative completes the form and forwards it directly to the Office of University Financial Aid, certifying the following information:

  • Details of the degree and program
  • Exact number of credit hours and tuition fees charged for the academic year
  • Type and amount (if any) of graduate financial assistance being received by the student through the department in question

This form is required of all graduate and professional students applying for aid at ÐÇ¿Õ´«Ã½, even if said student is not receiving any financial assistance from his or her department. If you are not certain that the form is going to be sent by the university department in question to the Office of University Financial Aid, please contact the department to arrange this.

Parent Asset Verification

Please only submit this form if it is listed as a required document on your My Financial Aid portal.

Proof of Citizenship

The Department of Education requires our office to verify some students' citizenship status. You are required to submit appropriate documentation, most often a birth certificate, certificate of naturalization or a valid U.S. passport, along with this document.

Proof of High School Completion

Please only submit this form if it is listed as a required document on your My Financial Aid portal. This form is requested when a Free Application for Federal Student Aid (FAFSA) is selected for a process called Verification that requires ÐÇ¿Õ´«Ã½ to collect documents to verify specific information from the FAFSA.

Proof of Permanent Residency

The Department of Education requires our office to verify some students' Permanent Residency status. You are required to submit appropriate documentation, most often a copy of your United States Permanent Resident Card (USCIS I-551), along with this document.

Request for Refund Advance

Complete this form and submit it to the Financial Aid Office if you are requesting an advance on your financial aid refund. This form is valid only for students enrolling or enrolled in the following schools: Undergraduate Colleges, School of Graduate Studies, Frances Payne Bolton School of Nursing, Weatherhead School of Management and the Mandel School of Applied Social Science.

Statement of Educational Purpose and Proof of Identity

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If you are notified by our office to submit this form, your record has been selected and you must submit the Statement of Educational Purpose and Proof of Identity form. This form must be brought into the Office of University Financial Aid in person; if a student cannot bring it in person, this statement must be notarized by a notary.

Student Asset Verification 

Please only submit this form if it is listed as a required document on your My Financial Aid portal.

Study Abroad Cost Sheet

This form should be completed by undergraduate students participating in the Study Abroad Program.

Verification Form – Dependent Students

Please only submit this form if it is listed as a required document on your My Financial Aid portal. The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. Students requiring this form should review the information (along with their parent(s)), and then complete and return it, as indicated on page 1. As part of the verification process, some students and their parent(s) are required to submit copies of their 2021 IRS Tax Return Form 1040 and W-2 forms as supporting documentation. Instructions on how to obtain the tax transcript is included in the form.

Verification Form – Independent Students

The Department of Education randomly selects a portion of FAFSA applicants for a process called verification. If a student is selected, he or she will have this document listed as a required document. Independent students requiring a Verification form should review the information (along with his/her spouse, if applicable), and then complete and return it, as indicated on page 1. As part of the verification process, students (and their spouses) are required to submit copies of their 2021 IRS Tax Return Form 1040 and W-2 forms as supporting documentation. Instructions on how to obtain the tax transcript is included in the form.

Unusual Enrollment History Form

Please only submit this form if it is listed as a required document on your My Financial Aid portal.
