Academic Advisement Report Substitution Form&²Ô²ú²õ±è;– Use this form to request a correction or course substitution on your Academic Advisement Report. Have form reviewed by your Academic Advisor and send to your four-year advisor for approval. 

Cross-registration Form&²Ô²ú²õ±è;– Use this form to register for a class via the Cross-Registration Program. The form must be submitted prior to the start of the semester. Send to ugadvisingsupport@case.edu.

‌Dean's Verification Form&²Ô²ú²õ±è;– Use this form to request a letter from the dean's office verifying your academic standing. Send to ugadvisingsupport@case.edu.

Major Declaration/Change Form&²Ô²ú²õ±è;– Use this form to declare or change a major. Complete form, send to Academic Representative, then send to ugadvisingsupport@case.edu

Minor Declaration/Change Form&²Ô²ú²õ±è;– Use this form to declare or change a minor.‌ Complete form, send to Academic Representative, then send to ugadvisingsupport@case.edu

Planned Program of Study Form&²Ô²ú²õ±è;– Students applying to a Combined Bachelor's/Master's (CBM) program must submit this form with their application materials.‌ Send to ugadvisingsupport@case.edu with approval of your graduate program advisor.

Drop/Add Form – Use this form to drop or add a class to your schedule. Send to your four-year advisor for review.

Online Submission

&²Ô²ú²õ±è;– Use this online form to apply for graduation through SIS (currently closed). If you need to apply for graduation and the online SIS form has closed, you will need to complete a paper Graduation Application and submit to ugadvisingsupport@case.edu.

Petitions and Requests

Off-campus Study Request Form&²Ô²ú²õ±è;– Use this form to request approval to take a course off-campus. Prior approval is required in order for transfer credit to be granted. Send to ugadvisingsupport@case.edu.

Petition for Re-enrollment After Separation&²Ô²ú²õ±è;–&²Ô²ú²õ±è;Use this form to request to return to ÐÇ¿Õ´«Ã½ after an academic separation.

Petition to Re-enroll After Voluntary Leave of Absence&²Ô²ú²õ±è;– Use this form to request to return to ÐÇ¿Õ´«Ã½ after you have taken a voluntary leave of absence.

‌Request for Voluntary Leave of Absence&²Ô²ú²õ±è;– Use this form to request a voluntary leave of absence from ÐÇ¿Õ´«Ã½. It is used if you are NOT enrolled for classes during the semester in which you are requesting the leave. If you are registered for classes, use the Request for Withdrawal Form.

Request for Withdrawal - Use this form to request to be withdrawn from all courses in which you are currently enrolled.

Special Request Petition (not available online, please see note) - Students who are requesting an exception to an academic policy must formally request this through the special petition process to the Undergraduate Advising Support Deans Committee. To obtain this form and start this process, students should contact their four-year advisor. The UAS Deans Committee meets regularly to review and decide on petitions. Please note that course substitutions and other curricular exceptions should use the Academic Advisement Report Substitution Form (linked on this page).

Transfer and Visiting Students

Visiting Undergraduate Student Application – Use this form to apply to be a transient/visiting student at ÐÇ¿Õ´«Ã½. Send to ugadvisingsupport@case.edu.

Visiting International Student Application (non-exchange)&²Ô²ú²õ±è;–&²Ô²ú²õ±è;Use this form if you are an international student and want to apply to be a transient/visiting student at ÐÇ¿Õ´«Ã½ during the summer, fall, or spring term. Send to ugadvisingsupport@case.edu.
